Our Mission
Lower Falls Pediatrics is committed to providing our patients with the highest standards of excellence in pediatric medicine from birth to 22. This is accomplished with evidence-based pediatric care, a special focus on development, and cutting edge technology. Please Bring to Your First Appointment
- Immunization records
- Medical records from your previous pediatrician. If you do not have these records, please fill out our Record Request Form
- Consent Form - One of these forms needs to be signed for each family in order for our physicians to care for your child in the event that someone other than yourself needs to bring your child to our office, e.g. a grandparent or a nanny.
- New Patient Questionnaire – Please try to complete prior to your first visit so that your doctor can review the information with you at the first visit. (please complete only if your child is over 2 years old)
