Prenatal Visit
The primary purpose of your prenatal visit is to establish an open, honest, and straightforward working relationship with your doctor. Our questionnaire attempts to gather background information on you and your family. You should consider these questions as the first level screening for your newborn's initial visit. Therefore, it is indeed extraordinarily important that you provide thoughtful and reflective responses.
Newborn Visit
Congratulations! We hope that labor and delivery have gone well for you and your newborn. The main purpose of this visit is the evaluation and assessment of your baby's health. Nevertheless, the parents overall well being is an important part of your baby's health. The responses to these questions will allow us to prepare an appropriate wellness and prevention care plan for our newborn patients and parents. Therefore, your attention to detail is greatly appreciated.
First Week Visit
The first few hours, days, weeks and months of your newborn's life are important medical evaluation periods. During this time frame, proper screening, evaluation, assessment and medical care can significantly improve the quality of life. You should consider these questions as the first level of health screen for your newborn's first week of life. Therefore, it is extraordinarily important that you provide thoughtful and reflective responses.
One Month Visit
Typically at one month, you begin to re-evaluate fundamental rules relative to injury prevention, routine home care and your baby's development. These clearly include considerations such as using an infant car seat, keeping constant surveillance on your baby, and what sort of help you need with caring for your baby. This questionnaire is designed to facilitate your thinking regarding the most appropriate care for you and your baby.
Two Month Visit
There are very important milestones that should be considered at two months. This questionnaire is designed to help you evaluate the feedback currently you are receiving from your two month old. It is also important for the parents to examine their own level of adjustment to having a new family member. Therefore, please consider each question carefully before answering.
Four Month Visit
By the time your infant is four months old, most working mothers are returning to work. It is important that childcare arrangements work for the baby and the parents. This questionnaire will help evaluate how you and your partner have managed the first four months of your child's life. Six Month Visit
Generally, a six-month-old baby wants to interact with people. The major developmental milestones of a six month old are social and emotional. We should expect smiles, laughs, and imitation of razzing noises. The purpose of this questionnaire is to make an assessment of developmental progression. The six-month visit questionnaire is designed to facilitate your thinking regarding the most appropriate milestones for you and your baby.
Nine Month Visit
It is important at nine-months that the parents' assess their attitudes and ability to cope with their child's separation and protest. We further want to continue the evaluation of your child's evolution. For example, your baby should now understand a few words such as "no-no" and "bye-bye". This questionnaire helps you make a very comprehensive evaluation of yourself and your baby. Therefore, it is extraordinarily important that you provide thoughtful and reflective responses.
